Wellness Program Participation and Liability Release Form

Acknowledgment and Acceptance of Risks:

I, hereby acknowledge that I have voluntarily chosen to participate in the MadB Energy Wellness Program offered by Brittany Reed. I understand that the wellness program includes, but is not limited to, nutrition guidance, exercise routines, mindfulness practices, and recommendations for supplements which are intended to promote general health and well-being. I recognize that these activities involve certain risks, including but not limited to physical injury, emotional distress, and other risks not yet known or reasonably foreseeable. I assert that my participation is entirely voluntary and that I assume all risks associated with such activities.

Medical Disclaimer:

I understand that MadB Energy Wellness Program and its representatives are not licensed medical care providers, dietitians, or nutritionists, and they do not hold themselves out to be such. I acknowledge that the advice and information provided in the program are not intended as a substitute for professional medical advice, diagnosis, or treatment. I affirm that I have been advised to seek the counsel of my physician or another qualified health care professional with any questions I may have regarding a medical condition, diet, fitness, or wellness program before beginning any diet, exercise, or supplement regimen, and before participating in this wellness program.

Liability Release:

In consideration of being permitted to participate in the MadB Energy Wellness Program, I, and my heirs, personal representatives, or assigns, do hereby release, waive, discharge, and covenant not to sue Brittany Reed, MadB Energy Wellness Program, its officers, employees, and agents from liability from any and all claims including the negligence of Brittany Reed, MadB Energy Wellness Program, its officers, employees, and agents, resulting in personal injury, accidents, illnesses (including death), and property loss arising from, but not limited to, participation in the MadB Energy Wellness Program. 

Acknowledgment of Understanding:

I have read this release of liability and assumption of risk agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
